We strive to automate routine tasks to alleviate the manual administrative effort required to manage patients.  With that goal in mind, we created logic in Centricity Practice Solution (CPS) and Centricity EMR (CEMR) to reduce the number of unmatched patients requiring manual reconciliation when processing electronic refill requests.  We are sending this notice to inform you of an issue recently discovered with the patient matching algorithm used when processing electronic prescription refill requests.  This issue was introduced during the CPS 12.2 and CEMR 9.10 release and all subsequent releases.

If your organization manages multiple patient charts for a single patient across multiple Locations of Care, then this issue may affect your organization.  If your organization does not manage multiple patient charts for a single patient across multiple Locations of Care, this will not affect your organization.

Soma Stout: Keynote Speaker CHUG Fall 2018Somava Stout, MD, MS (Soma)has dedicated her career to improving health, well-being and equity through the development of thriving people, organizations and communities.  She has worked as a primary care internist and pediatrician in the safety net and a global public health practitioner for over 20 years.

She currently serves as Vice President at the Institute for Healthcare Improvement (IHI) and as Executive Lead of 100 Million Healthier Lives, which brings together 1000+ partners in 28 countries to achieve transformation in health, well-being and equity, with an audacious aim: 100 million people thriving globally by 2020.  She serves as the co-Principal Innovator for the SCALE initiative, which supports communities around the country to accelerate their journey toward a Culture of Health through the development of Community of Solutions skills, behaviors and systems.

Previously, Dr. Stout served as Vice President for Patient Centered Medical Home Development at Cambridge Health Alliance (CHA), where she led a whole system transformation of leadership, care delivery, workforce, and finances that garnered numerous national awards and achieved breakthrough results in health outcomes and cost for a safety net population.  She founded and led the development of the CHA Revere and Everett Family Health Centers as national models for community-integrated primary care and helped to found the Everett Community Health Partnership.

Veritas Capital to acquire a GE Healthcare unit for $1 billion

General Electric and Veritas Capital, a leading private equity investment firm, announced today that Veritas will buy GE’s revenue-cycle, ambulatory care and workforce management software unit, which includes the value-based care division.

“Our team has significant knowledge and expertise in the healthcare IT space, and by operating as a standalone business under Veritas’ owndership, we now have the opportunity to further revitalize our product portfolio and pursue complementary acquisitions to better serve patients, providers, and payers,” Jon Zimmerman, vice president and general manager of value-based care solutions at GE Healthcare.

In her role at the Brookings Institution, Dr. Patel was instrumental in the development of several specialty payment models including the Oncology Care Model Initiative and the Next Generation ACO model.

Dr. Patel was previously a Director of Policy for The White House under President Obama and a senior advisor to the late Senator Edward Kennedy.  Her prior research in healthcare quality and community approaches to mental illness have earned national recognition and she has published numerous papers and book chapters on healthcare reform and health policy.  She has testified before Congress several times and she is a frequent guest expert on NPR, CBS, NBC and MSNBC as well as serving on the editorial board of the journal Health Affairs.

Dr. Steven Waldren, Director of the Alliance for eHealth Innovation of the American Academy of Family Physicians will discuss the impact of burnout on healthcare providers and ways to avoid it. Dr. Kavita PatelDr. Kavita Patel is a Nonresident Senior Fellow at the Brookings Institution and a co-founder of Tuple Health, a physician led company focused on practical clinical solutions to bring care back to health as well as a practicing primary care physician at Johns Hopkins Medicine. In her role at the Brookings Institution, Dr. Patel was instrumental in the development of several specialty payment models including the Oncology Care Model Initiative and the Next Generation ACO model.

Dr. Patel was previously a Director of Policy for The White House under President Obama and a senior advisor to the late Senator Edward Kennedy.  Her prior research in healthcare quality and community approaches to mental illness have earned national recognition and she has published numerous papers and book chapters on healthcare reform and health policy.  She has testified before Congress several times and she is a frequent guest expert on NPR, CBS, NBC and MSNBC as well as serving on the editorial board of the journal Health Affairs.

Provider burnout is at an all-time high. Two thirds of family physicians report at least one element of burnout. Some specialties suffer higher burnout rates than others. Many factors contribute including growing regulatory pressures, inefficient EHRs, emphasis on RVUs, poor workflows, and work-life imbalance. Dr. Steven Waldren, Director of the Alliance for eHealth Innovation of the American Academy of Family Physicians will discuss the impact of burnout on healthcare providers and ways to avoid it. We will then have an open forum with GE Healthcare exploring how Centricity can assist providers to lessen the burden of EHRs on burnout.

When in New Orleans, Po’ Boys are a must eat. Hurricane Harvey – Disaster Exceptions/Exemptions for Medicare Certified Providers Affected by Severe Storms & Flooding

The Centers for Medicare & Medicaid Services (CMS) is granting exceptions under certain Medicare quality reporting and value-based purchasing programs to acute care hospitals, PPS-exempt cancer hospitals, inpatient psychiatric facilities, skilled nursing facilities, home health agencies, hospices, inpatient rehabilitation facilities, outpatient dialysis facilities, long-term care hospitals, and ambulatory surgical centers located in areas affected by Hurricane Harvey due to the devastating impact of the storm. These providers will be granted exceptions without having to submit an extraordinary circumstances exception request if they are located in one of the Texas counties or Louisiana parishes, all of which have been designated by the Federal Emergency Management Agency (FEMA) as a major disaster county.

The scope and duration of the exception under each Medicare quality reporting program is described in the memo posted on 8-31-17; however, all of the exceptions are being granted to assist these providers while they direct their resources toward caring for their patients and repairing structural damages to facilities.

If FEMA expands the current disaster declaration for Hurricane Harvey to include additional counties or parishes, CMS will update this memo to expand the list of providers eligible to receive an exception without submitting a request to include the hospitals, PPS-exempt cancer hospitals, inpatient psychiatric facilities, skilled nursing facilities, home health agencies, hospices, inpatient rehabilitation facilities, long-term care hospitals, and ambulatory surgical centers located in the additional counties and parishes.

In addition, CMS will continue to monitor the situation and adjust exempted reporting periods and submission deadlines accordingly.

It’s no secret that the French Quarter is the place to party! With many clubs, DJs, and bars- the celebration opportunities are endless.  Though what’s often gone unnoticed is Bourbon Street’s history which dates back to the early 18th century.  The street is especially rich with historic venues, social tales and iconic buildings. “Rue Bourbon” is located at the heart of the French Quarter extending 13 blocks from Canal St. to Esplanade Avenue.  Regardless of the reason, your trip to New Orleans isn’t complete without a visit to Bourbon Street.

What to Eat: Gumbo

Enjoy some Louisiana Gumbo at CHUG Fall 2017Gumbo has come to be one of the best examples of the multicultural melting pot that has made New Orleans what it is. The dish is best described as a type of stew served over rice.

The challenges virtually every physician and caregiver faces using an electronic record motivates us to replace our newsletter this month in favor of an invitation to participate in an industry solution for EMR usability.

Your organization is invited to participate in a collaborative to improve the usability, effectiveness and happy adoption of EMRs. You and your peers have challenged KLAS to step up, leveraging our industry connections. You can help but it might take some extra courage.

The backbone of this approach is EMR utility and satisfaction measurements at organizations across the country. All physicians at an organization are invited to provide their two cents, allowing you and your organization to benchmark your users’ responses against those of other organizations. With the 13 organizations who have participated to date, we have begun to see this data be a powerful motivator for change and direction of what you can do to improve the experience of end users.

Quatris Health Announces Acquisition of HealthSystems

BEDFORD, TX – March 1, 2017 – Quatris Health (“Quatris”), a provider of specialized healthcare software and database solutions, as well as hosting, software maintenance and support services for small and medium sized physician practices, announced today that it has acquired Atlanta based HealthSystems. 7 Tips to Cultivate Your Trainer Part 2

 Tip #5:  Value-Added Quality

Add some polish to your curriculum and put some thought into how you can make this experience enjoyable for your students.  Ensure handouts are of nice quality and not bad photocopies.  Print a few extra in case you have unexpected attendees.  In your exercises and examples, use scenarios that make clinical sense.  Otherwise, your students may get lost in the details and your intended focus is lost.

Offering students choices is another way to add value and enhance the learning experience.  When I find wiggle-room in the agenda, I’ll offer the class the opportunity to choose what they learn next.  For example I’ll ask “Do you want to cover phone notes or patient check out next?” or, “Do you want to take a break or get out of class early?”  Giving them options makes them feel like they have some control.

In your exercises and examples, use scenarios that make clinical sense.  Otherwise, your students may get lost in the details and your intended focus is lost.

For a fun twist during class, give out treats as a small gesture to revive the group.  Using a theme for the class is another way to make it interesting.  One of my week-long training courses was a boot-camp theme that included MRE’s paper sacks containing military inspired treats, toy soldier name badges, and an American flag pinning ceremony at graduation.  The smallest things can make the class fun and promote interaction.  Awarding tickets for questions or participation then doing a raffle at the end of class with small prizes can go a long way for class participation.

As long as focus is not lost, put the fun back into learning.  Break up the seriousness of the day with a fun slide show review of the content and work-appropriate shenanigans.  One presentation that my class enjoyed was a gameshow-theme quiz.  Students like it when they hear or get to share relevant stories in the field. Approaches such as these make the content and workflows meaningful.

Tip #6:  Common Obstacles

In over a decade of training trainers, I’ve learned to share some strategies to keep pace of the agenda when class sizes are large.  Prepare your trainer to overcome hurdles with language difficulties and also to keep on pace when students lack basic computer skills.  Being patient while also respecting the students’ experience and history goes a long way.  New trainers are not often prepared to handle the disengagement of students during class.  One tip I use is to pause and say “You’ll want to write this down” and wait to move on as they write down each step.  I’ll recap the steps during regular intervals in case a student forgot to take notes.  When students are given the opportunity to follow along on the computer, you will have slow navigators.  Your trainer must be prepared with keeping the class on pace and accommodate learners of all skill levels.

A nurse, a front office registrar, and an office manager may all be interested in learning only the content that applies to their role. As the trainer, you know your agenda is relevant to all of them but you must find a way to engage every student.

More obstacles to prepare your trainer include methods to make the content relevant when training users from multi-specialties or various roles at the same time.  A nurse, a front office registrar, and an office manager may all be interested in learning only the content that applies to their role.  As the trainer, you know your agenda is relevant to all of them but you must find a way to engage every student.  Holding training away from the office and in an isolated training room is ideal for students to avoid disruptions from other staff.

Another challenge is managing a class when side-bar conversations take place.  Sometimes, the conversation is relevant, while other times it is not at all and is causing a disruption. One technique is to stop and ask the group if they have a question since the class may also be wondering about the same thing.  If it is relevant, allow for an open discussion to engage the class.  If the conversation goes off topic, put it on a parking lot to come back to later.

Not everyone comes to class, sits in the front row, and enthusiastically takes notes to learn.  There will be times when your trainer must deal with negativity, disinterest, or shutting down.  In the past, it has been helpful to involve negative users and make them a part of finding solutions.  My experience has taught me that negative people are also passionate about what they do.  When they are part of the solution, they become a powerful advisor and often get buy-in from their peers when a solution is found.

Tip #7:  Mentoring, Etiquette, Work Ethic and Growth

One of the best approaches I’ve experienced is allowing your trainer the opportunity to shadow a mentor and be shadowed.  This dynamic provides a coaching environment where your trainer can gain confidence.  A mentor can provide valuable feedback for how to handle questions that come up in class while also giving the trainer room to learn and grow independently.   Your trainer may learn from the mentor and also be encouraged to get involved outside of the classroom.  For example, a trainer may gain value by joining an industry association or club or through networking via an online forum.  Remind the trainer that when networking outside of the classroom, you still represent your organization and yourself.  Project a nice appearance to gain credibility while being yourself and still a professional.

An effective trainer is more than someone who shows up to communicate material.  They must take initiative and advantage of their own learning opportunities to grow.  Putting the content into context, it is helpful for a trainer to visit practices onsite, speak to support about common support calls, and work with subject matter experts about workflow details.  All of this information can be brought back to the classroom to enhance the delivery for the next class.

An effective trainer is more than someone who shows up to communicate material. They must take initiative and advantage of their own learning opportunities to grow.

For leadership, set realistic goals and expectations for your new trainer.  Base your expectations on the trainer’s prior experience and give them time to not only learn the material, but to learn to train it.  Your new trainer is faced with double duty.  As the trainer, you are not expected to know every answer.  Finding out answers to questions is a great way to learn and will take time.

As a trainer, use test scores to reflect on your training and improve the next time.  Offer surveys to collect essential feedback for content.  Expect some negative feedback but use it as an opportunity to improve your delivery and style.  Along those lines, learn to identify a student who is clearly not absorbing the material.  Communicate with managers or leadership when needed so they can best address the situation.  As a trainer, you are not tasked with qualifying candidates, for hiring, or firing.  But you do have a responsibility to ensure a productive and positive learning experience.

On a personal note, understand that all students may not like you.  Be confident in the fact that training is not about being liked, it is about facilitating learning and ensuring each student is provided the opportunity to learn skills needed to perform their job.  Alternatively, it is important to recognize that not every personality makes a good trainer.  A trainer must have the skills needed to adapt to the class energy and environment.  Trainers are sometimes placed in this role because they are good at using the software.  Proficient software skills does not mean a person is equipped to train.  It takes a specific personality to deliver the content and instruct users successfully.

The next piece of advice is to not overload your trainer.  Give your trainer a break when possible by allowing them to work from home, letting them go early, or give them a change of scenery by shifting to a support role for a day.  Training is mentally draining.  In return, trainers must learn their bandwidth limits.  It is in our nature to want to help but instructors must learn their thresholds and speak up when they are overwhelmed.  Failure to communicate will lead to burnout or low quality work.

Success in motion:

A recent project in Austin, Texas inspired me to share my collection of tips and tricks.  While providing interim training for new employees for a large organization, I met an energetic young lady who was preparing to take over the instructor role.  As a new employee, she was tasked with learning a new software and how to train the workflows simultaneously.  She quickly shadowed and taught the class while being shadowed.  We met online between training weeks to iron out any details and refine the training material.  This approach gave her independence with a safety net.

Above learning the material, she took a genuine interest in being the best instructor she could be.  Because of this, I wanted to nurture her enthusiasm with some “extra” mentoring.  Working together we evaluated the current curriculum and revamped the material.  The new trainer worked on her presentation techniques and mastered the software in record time.  Towards the end of my engagement, this young professional was off and running in record time due to her work ethic, technical ability, professional attitude and desire to learn.

As you evaluate your own training program, consider all of the elements to deliver a truly impactful learning experience that will translate into a return in your investment in your employees.  Knowledge is truly a gift.  Why not wrap it up with a nice bow?

CMS Extends Deadline for 2016 Physician Quality Reporting System (PQRS) Electronic Health Record (EHR) Submission

CMS is extending the submission deadline for 2016 Quality Reporting Document Architecture (QRDA) data submission for the EHR reporting mechanism. Individual eligible professionals (EPs), PQRS group practices, qualified clinical data registries (QCDRs), and qualified EHR data submission vendors (DSVs) now have until March 13, 2017 to submit 2016 EHR data via QRDA. The original submission deadline was February 28, 2017.

STEADI Fall Risk Assessment Form

The Centers for Disease Control and Prevention (CDC), American College of Preventive Medicine (ACPM), a team of national experts, and PatientLink worked together to design and build a free fall risk clinical decision support (CDS) encounter form. This tool will help you incorporate fall risk assessment and fall prevention into your clinical practice – and enhance your efforts to help older adults stay healthy and independent. Clinical Decision Support Tool to Decrease Fall Risk in Older Adults

CDC Article Falls are preventable and can be considerably reduced if high risk patients are identified through screening and receive appropriate follow-up care.

If your practice serves adults 65 and older, you should already be doing fall risk assessments. However, many doctors don’t due to time constraints.

While time is limited at an appointment, it’s crucial for doctors to help patients develop a plan to decrease their fall risk. The Center for Disease Control and Prevention (CDC) recommends that doctors incorporate fall prevention into their regular practice. Doctors should be informed on what they can do to prevent falls among their older adult patients, such as recommending vitamin D, reducing medications that might increase falls, and referring patients to community programs or physical therapy to improve their balance.

“I continue to use the tool in my daily practice.”

The CDC developed the Stopping Elderly Accidents, Deaths and Injuries (STEADI) initiative to make fall prevention a routine part of clinical care. STEADI includes a clinical algorithm, adapted from the American and British Geriatric Societies’ Clinical Practice Guideline, which helps sort patients by fall risk level. Although doctors found the algorithm useful, they wanted it integrated into their Electronic Health Record (EHR) systems. The CDC partnered with the American College of Preventive Medicine and PatientLink to create an EHR Clinical Decision Support Tool based on the STEADI toolkit that would work within the GE Centricity EHR.

Creating a Clinical Decision Support Tool to Assess Fall Risk

A national team of doctors and researchers set out to create the content of the tool, and worked with PatientLink to build it. The team met regularly to review what Debi Willis, technical engineer on the project and owner of PatientLink, was building and to provide feedback through the entire process.

In the first stage, PatientLink created a tool based on the complete CDC STEADI algorithm. Once ready to be tested in a real-life setting, PatientLink connected with physicians at Oklahoma University (OU) Medicine to test the tool. It was integrated into OU primary care practices where it was evaluated for its usability, technical soundness, convenience and modified based on feedback from doctors.

Dr. Robert Salinas, family physician and geriatrician at OU, was part of the national advisory committee and also the lead physician in testing the tool within Centricity. He found the tool to be incredibly helpful. “I continue to use the tool in my daily practice,” said Dr. Salinas. “It helps me and my patients create an easy-to-follow plan for optimal care.”

The tool has multiple sections, divided into tabs for easy toggling. The first tab is the patient’s 12-question self-assessment, which they can fill out prior to entering the office. Based on their answers, the EHR tool auto calculates a fall risk score for the doctor. If the patient is at increased risk for falls, further assessment and preventive measures are recommended, which are facilitated by the EHR.

CDC ArticleFor those that fail the initial screen, the doctor is guided through tabs including assessments (e.g., gait and balance), medication review, and a physical examination and plan of care tab, where the doctors can perform additional assessments if needed and develop a plan for follow-up care. For instance, if the patient had poor muscular strength, the doctor may suggest physical therapy. Then, the doctor can plan to meet with the patient again in six weeks to observe improvement and hopefully find that the patient has better balance and is at a lower risk for falls.

Dr. Salinas shared that not only did he and his fellow doctors enjoy the tool’s ability to better assist and assess for fall risk, his patients appreciated the tool, as well. The patients interviewed provided positive feedback and felt the doctor really cared and wanted to help, versus only asking questions and moving on regardless of the response.

After the first-round testing phase was complete, the doctors confirmed the tool was very helpful but had one overriding recommendation. They wanted the tool to automatically identify which of the patient’s medications might affect their fall risk.

Updating and Improving the Clinical Decision Support Tool

During the second stage of development, the national team got together to identify the medication categories that were associated with higher fall risk. The team wanted to provide doctors a way to easily identify whether their patients were taking medications that increased their risk of falling, in order to assist them in determining whether these medications should be stopped, switched, or reduced.

Once the new tool was completed, the team sent it back to the doctors, who tested the tool with more than 500 patients, providing multiple rounds of feedback to the software development team along the way. The doctors found the new tool to be very useful. The only remaining problem was the time needed to fully assess a patient for fall risk and recommend interventions.
Fitting fall prevention into a typical office visit remains a challenge. The Centers for Medicare and Medicaid Services (CMS) encourages fall screening by making it a component of the Welcome to Medicare Visit and the Medicare Annual Wellness Visit; however, these visits are not universally used and fall prevention is just one of many parts. Falls-related quality measures are also included in CMS incentive programs which provide an additional incentive for fall prevention.

“We certainly hope that a lot of doctors will use this tool and find it useful,” said Erin Parker, PhD, Health Scientist at CDC. “We know that doctors are aware of falls in older adults and want to help but don’t have all the needed resources, but now they do. We want them to use this tool and help patients decrease their risk.”

More Reasons to Perform Proper Risk Assessment

The numbers provided by the CDC speak for themselves:

    1. Falls are the leading cause of fatal and nonfatal injuries among older adults (aged 65 years and over).
    2. Every second of every day in the U.S. an older American falls
    3. 1 out of 5 falls cause a serious injury such as a fracture or head trauma.
    4. In 2014 over 27,000 older Americans died because of falls, 2.8 million were treated in emergency departments (EDs) for fall-related injuries and >800,000 of these patients were subsequently hospitalized
    5. By integrating fall prevention into clinical practice physicians have the potential to reduce future falls by nearly 25%.

Physician Burnout and Professional Dissatisfaction Linked to Electronic Health Records and Computerized Physician Order Entry

Use of electronic health records (EHRs) and computerized physician order entry (CPOE) may lead to lower physician satisfaction and higher rates of professional burnout, according to large nationwide study published in the July issue ofMayo Clinic Proceedings.

“Electronic health records hold great promise for enhancing coordination of care and improving quality of care,” said lead author Tait Shanafelt, MD, Director of the Mayo Clinic Department of Medicine Program on Physician Well-being. “In their current form and implementation, however, they have had a number of unintended negative consequences, including reducing efficiency, increasing clerical burden, and increasing the risk of burnout for physicians.”

IMPORTANT JULY KB NOTICE!

Dear Centricity customer,
This morning Centricity Support found an issue with interaction checking that arose after applying the July 2016 Incremental Knowledgebase This issue was found to affect both EMR and CPS products.

We are asking customers who have downloaded the July 2016 KB zip file but have not installed it, please do not install. We are working with Engineering to identify the cause of the issue and will release a new build when the issue is fixed.

GE Healthcare Announces Project Northstar

Jon ZimmermanGE Healthcare announced this morning at its Centricity Live 2016 user conference in Phoenix, AZ its next-generation IT solution for ambulatory care delivery. I spoke with GE Healthcare IT VP/GM Jon Zimmerman of the company’s value-based care solutions team ahead of the announcement.

Describe Project Northstar that is being announced.

Project Northstar is GE Healthcare’s next-generation IT solution for ambulatory care delivery to fundamentally help practices thrive in the world of value-based care. We strongly believe that the move to value-based care is on. It’s not going to be a light switch. It will be a transition over time.

We also see that the tools and services that have been built around population health have not been integrated with care delivery from a community perspective. It’s certainly not completely integrated with revenue cycle management with both value and volume in mind. Payers are changing, too, so there’s new payer connectivity required.

We’re taking a point of view from a physician’s workflow and driving population health integrated with care delivery, integrated with revenue-cycle management both value and volume, with new forms of payer connectivity to take waste out of the system. Our drive is to increase quality, efficiency, and financial performance for customers.

Is this a standalone product or is it just for Centricity users? Who is the target customer?

The audience starts with GE Centricity Practice Solutions / GE Centricity EMR first, but we built it with open principles. We believe that some of the advanced ACOs may want to take some of the capabilities that we’re offering and also make them useful and integrated on top of other EMRs.

Is it an upgrade or a separate product that Centricity customers will buy?

Look at it as an extension from what people have today with a migration path to roll it over Centricity over the next few years. We believe that a big-bang replacement would be a very bad and disruptive idea. Many of our customers have given us great clues on how to do a safe, smart migration transition. It’s not a big bang, turn that off, turn that on.

What providers and partners did you work with?

We worked with Westmed Practice Partners in Westchester, NY starting almost two years ago. One of the things that was highly attractive about working with Westmed is that they were scoring very high in their quality measures. Their efficiency measures and their ability to collect revenue from their fee- and value-based contracts were also very good. Their leadership knew what they were doing. They knew how they did it.

They were pushing our products up to and beyond their capabilities in order to make that happen. When I thought about how we were going to get to that next generation and who we could work with, I thought it would be good to start with somebody who was so very skilled and who knew us so well. That was Westmed Practice Partners, specifically Dr. Simeon Schwartz, the chairman and CEO.

Was the product built from scratch?

It is not being built from scratch, nor is it being acquired. This was a big discussion that Simeon and I had in the beginning. We are building certain components. We’re also assembling capabilities from different technology providers across the industry.

I don’t think anybody is going to be able to have the time to just go build from scratch, but taking a modern, 21st-century approach is going to be key. We have the luxury of leveraging is a lot of the investments that GE is making with Health Cloud, so this is an extension of what GE is doing as well.

It seems that you’re picturing an ecosystem with components provided by partners. How will that look?

First and foremost, we took a tabula rasa approach, meaning a blank slate. Once we got comfortable with one another in Westmed – and other practices have also helped us design this — one of the keys was, how do you guys work? What do you do all day? We went even to the depths of, with appropriate permissions,observing their delivery of care.

We broke it down with a number of usability experts. GE Corporate, GE Digital has been investing in usability expertise and usability engineers. We leveraged those to break down the work processes of a pretty complex multi-specialty practice. We also focused strongly on, as you would imagine, that primary care is the quarterback, and that user experience is a big deal.

On the business side, we said, how does that work? How can we make a system provide more value for the providers? We broke the providers’ work into basically four areas.

Number one is that I need to understand. When I’m going to see a patient, I need to know a lot about them. How should a system gather that information for me?

Once it gathers that information, I need to know what I’m supposed to do. I need to know how to work. Underneath that is a rules engine that we’ve selected. The rules will be based on what the clinicians want to do. We’ll get rules from specialty societies or individual practices and combinations thereof. They will create a rules-driven system that’s based on a modern user experience with workflow guidance to then get the providers to do what the providers know that they need to do. Our approach here will remove clicks, but also provide consistency through the guidance of the decision-makers for that practice.

The next piece of work is that I need to review and sign. Rules comes in and say, did I do all the things that I’m supposed to do that will be impactful for my volume-driven revenue cycle? Did I document what’s required for my quality reporting?

Last but certainly not least, there’s follow-up care coordination and care management that creates a continuous loop in the system versus a set of independent acts.

For the user experience, we’re using the same technology that Google uses. That’s called AngularJS. For the rules engine, we’ve purchased a commercial rules engine and we’ve put that into our stack. To fill the rules engine, we’re working with a number of practices, with Dr. Schwartz being the first. We have another one signed up specifically for cardiology. We have a workflow engine. Our cloud provider is technologies from GE Health Cloud and supplemented by some things we’ve been doing with Microsoft and Azure over the last few years.

You mentioned reduced clicks and the user experience. You’re not replacing the UI of Centricity, correct?

We had a lot of robust dialogue with our customers on this. The first and greatest impact that we can have is the process of creating intelligent orders — orders that take the context of the patient, the context of the payer, and the context of evidence-based practice and build them into one.

In our initial implementation, users will be in Centricity up to the point where it’s time to create an order. Then the new system takes over seamlessly. It pulls all the information that customers are used to in Centricity. Now you’re into the cloud experience, the next-generation system. Once you complete that set of tasks, we bring you back into the world that you live in.

Physicians spend an awful lot of their time, as they should, in workflows for ordering and diagnosing. That’s why we did that. The more that we talk with customers, they said, “You made absolutely the right choice.”

How are you using payer information?

I was one of the lucky people who got to work as a pioneer to invent what we know today as the EDI systems of the US for healthcare starting back in the late 1980s. I have a long-term relationship with working between payers and providers. Just before I came to GE, I was lucky enough to work with a great company called Availity, a provider / payer network owned by 21 Blue Crosses and Humana. I got the opportunity to understand a lot of the payer processes and what’s missing in the bridge between payers and providers that creates an awful lot of wasted work.

GE was an inaugural investor in the AHIP Innovation Laboratory. AHIP, the payers’ professional association, knew that they had to create more innovation because of the trends that we see. We are inaugural investors.

We are reverse engineering the exchanges of information between payers and providers that goes through phone calls, faxes, physical mail, and portals and embedding that into our current and next-generation systems. A very important point: this is not going to have to wait for a next generation. We’re doing that now.

Let me give you a couple of examples. In Medicare Advantage, being able to prove as a payer that you are closing gaps in care and that patients are getting good care requires that if the payers see that things are not happening at differential analysis, then we can take a gap in care directly from a payers’ system. Some are pushing them out through sidecars and eligibility transactions. We put that information into the providers’ workflow so the know what’s necessary to be done. Then the providers can use their normal processes to get the work done and deliver the care.

Then payers are going to want it reported back. They’ll take it through a claim, or some are asking for CCDAs to be sent to them. We also are building the capabilities to deliver the clinical care documents, then the summaries with details, back to the payers so they can ingest them into their various systems, not their claim systems.

Another example is the need for hierarchical condition categories for risk adjustment. We can construct the appropriate data sets that payers are constantly calling the providers for and we can deliver it to them electronically. We know this because we work directly with payers and providers in their distinct workflows to be able to build these new bridges, to do it as electronically as possible within the workflows to reduce burden, reduce waste, and deliver on the Triple Aim.

What’s the timeline for delivering the product?

The first wave is going to come out in Q1 ’17. We’re working with our user groups and providers directly. There’s that preparatory. Then the orders module will come out first, followed by more enhancements that we’re going to be delivering in the RCM, followed by more and more clinical documentation and a collaboration. We’re also simultaneously building a lot more interoperability for collaboration among providers.

Everything that we’re doing from a workflow and technology perspective is being supported by a cloud-based interoperability collaboration hub and supported by analytics that are integrated as well, because there’s going to be a lot of adjustments over time.

We see this complete picture rolling out over the next three years. Based on demand and based on the number of ecosystem partners that we see, we hope to be able to accelerate that, but we want to first and foremost do no harm and create a lot of value as people have to change their business models during this very dynamic time, like none other that we’ve seen before in this industry.
