Meaningful Use Stage 2 Tips: Core Measure 12 (Preventive Care)

To meet Core Measure 12, Centricity has the ability to mass generate letters in conjunction with Inquires to transmit targeted reminder letters. It is possible to retrieve a subset of patients with a certain condition who were seen on a given date range. After retrieving the list, reminder letters can be generated for each patient, pulling patient specific details into the letter template. To have the action counted with CQR, the letter MUST be saved in a folder with “Actionable” in the title.

Example: To setup an inquiry to retrieve diabetic patients seen in a month, an inquiry can be setup with these criteria:
FIND patients WHERE problem code ICD-9, active CONTAINS ’250′ AND FIND patients WHERE appointment date IS ON OR AFTER 01/01/2015 AND FIND patients WHERE appointment date IS BEFORE 02/01/2015.

After clicking search and retrieving the information, reminders can be generated by selecting the activity of SEND REMINDER. Find the letter template and click print.

ICD-10 WEBINAR SERIES: Prepare Now for a Smooth Transition

PQRS UPDATE: Simplified workflow for 2014

Customers who have not yet finished applying SOP codes may now use the following workflow.
Part 1: Please review the following required and optional steps

(a) Assign initial SOP codes:
1. Required:  Apply Medicare FFS SOP code.
2. Optional:  Apply any other non-Medicare SOP codes.
Remember, if you have already applied any other Medicare codes (including “Medicare Non-managed Care other” code), we recommend you map those to “Unavailable/Unknown” before proceeding.

(b) Send data to CQR (via new script to reset subscription) and Calculate.
(c) Filter by Insurance: Medicare.
(d) Select measures to meet the CMS Medicare FFS reporting guidance.
(See instructions in Measure Selection & Provider Authorization webinar slides and recording.)

Part 2: Please review the following required and optional steps.
a) Optional: Assign remaining SOP codes:
1. Apply remaining Medicare SOP codes.
2. If in Part 1(a) you had mapped non-FFS Medicare plans to “Unavailable/Unknown”, you may re-map those to the appropriate non-FFS Medicare code before proceeding.
b) If you performed Part 2(a), send data to CQR (via script to reset subscription) and Recalculate.

BREAKING NEWS: CMS relaxes PQRS 2014 requirement to apply all SOP codes

Dear Quality Submission Service Participant:

We have some important news about the PQRS 2014 SOP code program requirements that we believe will provide significant relief for customers who are still in the process of applying SOP codes. This news includes updated guidance from CMS in addition to the creation of a new CQM script.

RELAXED SOP CODE REQUIRMENTS FOR 2014

GE Healthcare has been working with CMS over the last months to clarify the application of SOP codes for health plans. In our recent communications with you (webinars on 1/22 & 2/3; email on 2/4), we outlined GE’s interpretation of the PQRS 2014 requirements. CMS confirmed our interpretation but also acknowledged that there has been confusion among vendors in the use of the detailed SOP codes for PQRS submissions.

Yesterday, CMS stated an important change to the PQRS guidelines that adds customer flexibility:

• It is now acceptable, at your discretion, to ONLY apply SOP codes to the Medicare FFS (Part B) plans for PQRS 2014 reporting and to categorize the remaining health plans in a single broad health plan “Category D – Other.”
• Our customers may leave the non-Medicare FFS plans uncoded and these will be mapped by CQR to broad health plan “Category D – Other” without any intervention on your part.
• Please see the attached document for details on applying this guidance.
Please note: Our guidance to report on at least 9 measures across 3 domains that each represent a significant Medicare FFS initial patient population remains unchanged.

CUSTOMERS WHO HAVE ALREADY FULLY COMPLETED THEIR SOP CODE MAPPINGS:

CMS advised us to inform our customers that the requirement to apply SOP codes for all payers will be reinstated for PQRS submissions for the 2015 reporting year. Therefore, if you have already fully completed your SOP code mappings, according to our previously issued workflow for the 2014 program year, rest assured that you do not need to un-map any previously assigned SOP codes with this CMS clarification. You are well positioned for the 2015 program year when the requirement goes into effect.

New “Reset CQM Subscription” Script 

In addition to the welcome news from CMS, we are pleased to announce the availability of the “Reset CQM Subscription” script, as discussed on our February 3rd. CPS and CEMR customers will be able to download the script from the service portal once it is posted later today. After applying SOP codes in Part 1 (a), you must reset the Clinical Quality Measures subscription to transmit the data to CQR, and this new script automates that subscription reset process.

We welcome yesterday’s response from CMS and hope this news helps you more easily meet the requirement to authorize your providers by February 13, 2015. Schedule Right the FIRST Time

We use a Triage Scheduling form in our EMR to help guide staff into doing the right thing when scheduling. Scheduling staff are asked to follow so many different scheduling preferences it can be overwhelming trying to remember them all. We took the “remembering” aspect out of it and built our Triage form to do the remembering for us.

Staff enter information such as; type of injury, date of injury, insurance information, body part, etc., the EMR calculates that information along with age and zip code and will auto generate a list of appropriate physicians that I can schedule with. The form can give “pop-ups” that help with specific preferences such as “30 min for all bilateral hips” or “prior authorization before scheduling 2nd opinions”.

  • Service Pack 7 is now generally available. This release supports 14 additional Clinical Quality Measures (bringing the total of supported CQMs to 36), it includes PQRS updates to identify Medicare Part B patient visits, and it incorporates the MU Functional Measure changes originally available only through the Meaningful Use Update Patch.
  • We published a revision to the Meaningful Use Workflows Info Sheet that describes new workflows and resolutions to help ensure you capture the appropriate data for calculations.
  • Finally, we released an updated version of the Meaningful Use User Guide for Centricity Practice Solution and Centricity EMR customers that will serve as your comprehensive resource for understanding how the measures are calculated and workflows that support those measures.

Increase Productivity with Letters

Insurance Companies have never made claims processing a simple task and with ICD10 this is likely to get worse and not better. Now is the perfect time to reevaluate your accounts receivable department, analyze their workflow, and increase productivity.

Utilizing letters within CPS is one way to make the most of staff’s precious time. How often does your staff need to complete appeal letters in order to be paid for services? This is a frequent task at our site and one that previously took so long it was pushing aside other necessary tasks. Staff workflow included pulling the carriers appeal form off their website and manually completing it, too much time was going into ‘double data entry’ all the information is listed in CPS, yet staff were spending time copying it onto the insurance companies form.

Schedule & Manage Surgeries with your EMR

Have you considered using your EMR to schedule and manage your surgeries?

  • Create an encounter form to help your provider order surgery.
  • Use required fields to make sure the provider cannot advance without supplying the minimally essential information needed to schedule the procedure.
  • Program dynamic lists into the form to be specific to each surgeon. Only show them the special surgical needs they use or would like to see.
  • Formatting on the order page will help organize this surgical information in a structure that is easy for the receiver to read. Critical information can be placed near the top of the order followed by supporting documentation, or a particular detail can be placed in all CAPS. Either way the significant information will stand out.

Successfully Implement a Patient Portal!

When I started to implement a patient portal there were a few things I realized I needed to think about first.

1. Hardware and bandwidth:

One of the first things you have to consider is where is it going to run, on your own equipment or hosted? If in house, you will need a dedicate server (or two) depending on how many providers you have and what you expect in usage. This will use your internet connection, so make sure you have enough bandwidth to serve your practice plus additional for patient portal use. If you are using a hosted solution for your EMR and patient portal, you mainly have to worry about the cost of the additional servers and the bandwidth from your provider. Do not forget that your usage will increase over time!

2. Design:

Second thing to do is to figure out your design of the patient portal. If you have an existing website trying to mimic it can be difficult, but not impossible. Think on what will make easy to use to for your least technical patient. You want to make sure that they understand where to go to send messages, or fill out patient forms. Once you have settled on your design, start to fill in with content.

Eight Strategies to Improve Your Training and System Adoption Rates

1. Establish backups for people who are in training to maintain productivity, temporary staff, or a colleague who is cross-trained in the user’s work. It is difficult to focus on training when you know your work is piling up!

2. Utilize multiple training tools, because everyone learns differently supplement classroom training with computer-based training (CBT), web training, workbooks, etc.

3. Trainers LOVE lists of questions! Make the most out of the time spent training, have participants do some pre-work before class, so that they come to class with questions. Also, have the administrative tasks taken care of prior to training (logins, passwords, etc.)

9. Three years of increasingly responsible experience in a large, corporate systems development and implementation activities in a wide variety of Healthcare Management Information Systems (MIS) technical experience in system analysis, design, building, testing and implementation preferred.

PRINCIPAL ACCOUNTABILITIES:
1. Technical Administration – Responsible for providing technical knowledge in the support of software applications and various system environments. Perform the analysis, design, building, testing, and implementing of computer programs and systems. Demonstrate and apply information systems theory, processes and methodologies. Reason: To effectively provide the technical skills required to successfully support system programs and applications that meet the technical requirements defined by MH. Apply appropriate theories and procedures that meet the business objectives of the company.
2. Planning & Coordinating – Develop, prioritize and execute projects according to established schedules and timeframes. Maintain an effective level of coordination on all assigned projects. Document and report on all plans, priorities and schedules. Inform Manager on status of all projects and assignments in the form of weekly activity reports. Reason: To ensure an effective method of planning and coordinating project management, as well as ensure that projects are completed according to set schedules, and reported to management on a regular basis.
3. Systems Development – Translate user systems needs into alternative technical solutions. Perform project reviews and process walkthroughs incorporating technical and business solutions that will enhance user productivity and department service levels. Reason: To provide the technical skills required to successfully design, develop, deploy and maintain the technologies needed to satisfy user and applications strategies at a minimum cost and with maximum effectiveness.
4. Technical Standards – Act as an advocate for setting and enforcing technical development, application support and information management standards within and outside of the information systems department. Reason: To ensure that standards are established and followed consistently for the benefit of the company.
5. Application Support – Demonstrate in-depth knowledge and techniques in supporting Application Analysts and Senior Application Analysts in development projects and complex problem solving efforts. Participate in after-hours technical support by rotating call schedule on a regular basis. Reason: To provide resolution methods and alternatives to problems that can jeopardize the processes and workflow of MH's systems. To provide technical solutions to critical issues.
6. Training – Prepare and conduct training sessions for staff. Reason: To ensure staff is trained and knowledgeable in the use of computer programs and systems.
7. Customer Service – Provide effective Customer Service by being courteous, polite and friendly toward others at all times. Acknowledge customers immediately in order to determine need and help the customer resolve issue or request. Reason: Consistently display behavior that enhances the image of organization as evidenced by professional and courteous behavior toward all customers.
8. Other – Perform other duties and special projects as assigned by Manager or Team Lead. Adheres to all MH policies, procedures, and standards, within budgetary specifications, including time management, supply management, productivity, and accuracy of practice. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff.
